Leading off the 10th against fellow Dominican Felix Bautista, Guerrero singled into left field to drive in automatic runner Bo Bichette from second base.
Guerrero went 3-for-4 and scored three times. Toronto got its 15th win in 20 games.
Matt Chapman and Teoscar Hernández each hit two-run home runs in the third inning.
Adam Cimber (7-2) pitched an inning of relief for the win.
