Cleveland reliever Trevor Stephan starting the ninth inning, gave up a walk, a single and an RBI double to Jonathan Schoop that pulled Detroit within 3-2.
Enyel De Los Santos (3-0) took the mound with one out and gave up a grounder to Eric Haase that produced the tying run on a choice play.
Bryan Shaw pitched the 10th inning and earned his first save of the season.
The Guardians, leaders in the American League Central Division, increased their lead to a game and a half over the Twins, who had the day off.
